A blameless incident report: severity, status and a duration worked out from the start and end, the summary, impact with figures, a timeline marked with detection and response and elapsed time from the start, root cause with an optional five-whys chain and contributing factors, what went well, what went poorly and where you got lucky, an action item table and lessons.

Build with the "Incident postmortem" template from my Super Artifacts library.

After installing, ask me three short questions — one message, numbered — that fill the template's required slots. Wait for my answers, then build one artifact from them.

The questions that fit this template (three at most in the message — merge the fourth into the third, or drop it if you can infer it): (1) what happened in my words, and when it started and ended (with the time zone); (2) where the record is — an incident channel export, alert history, status page updates, deploy logs — and read it rather than asking me to retype it; (3) who needs to read the report (the team, leadership, customers), which sets how much detail to include; (4) which actions are already agreed and who owns each, as a team or role. Then draft the report: summary, impact with the figures I can support, the timeline with detection and response marked, root cause and contributing factors, and the action table. Write every actor as a role, never a name. Show the draft and mark it as a draft until I have reviewed it.
